When you need a little time to heal

Sometimes you need more than a few days in the hospital to recover from surgical procedures and other medical events and going home is just not an option for you. For example, you might have had joint replacement surgery, or a surgical procedure that could not be performed as an outpatient and you need therapy before you can go home and return to your normal activities. We are here to help you get to the next step towards going home and getting back to your normal activities. We also care for patients with chronic conditions who need a little extra time to manage their illnesses at home. We take a patient centered approach where our focus is safety, quality, and building strong relationships with you and your loved ones.

Our Skilled Nursing Facility

The Extended Care Unit is located on the 3rd floor of Wythe County Community Hospital. This 8-bed skilled nursing facility offers care to short-term patients with access to hospital and physician services. It allows for patients to receive a unique continuum of care. If home health or other outpatient services are needed when you are discharged from our extended care unit, our patient care managers will work with you to make sure you receive those services.

We provide short term services in addition to specialty services such as physical, occupational and speech rehabilitation.

Our Extended Care Unit is 5-Star Rated from The Centers for Medicare & Medicaid Services (CMS). CMS created the 5-Star Quality Rating System to help consumers, their families, and caregivers compare nursing homes/skilled care facilities more easily and to help identify areas about which one may want to ask questions. The Nursing Home Compare Web site features a quality rating system that gives each nursing home a rating of between 1 and 5. Skilled nursing care units with 5 stars are considered to have quality much above average quality.
